Sex disparities in psychosocial distress among medical students with indirect exposure to the 2023 Kahramanmaras earthquakes-commentary on previous paper

The Null Results: Interpreting Absence of Evidence

The failure to find robust sex differences is itself informative. The existing literature on direct exposure consistently shows women reporting higher anxiety, depression, and PTSD symptoms. This study’s null findings suggest that the mechanisms producing sex disparities in post-disaster mental health may require direct threat exposure or resource loss—not merely vicarious threat. The conservation of resources theory predicts that resource loss spirals are initiated by actual or threatened loss; perhaps remote threat is insufficient to trigger sex-differentiated responses of the magnitude seen in directly exposed populations.

However, two alternative explanations deserve consideration. First, the small sample (particularly the small male subgroup, n = 42) may have been underpowered to detect moderate effects. The authors note several raw associations approaching conventional significance (smoking, p_raw = .008; anxiety, p_raw = .083) that could represent genuine population differences. Second, medical students represent a selected population with potentially higher resilience than the general population—they have succeeded in a competitive admissions process and may possess coping resources that attenuate sex differences.

Ethical and Reporting Considerations

The paper includes an AI usage disclosure stating that ChatGPT-4 was used for language assistance and figure generation. The authors assert they reviewed and validated the output, which meets current transparency standards. However, the use of AI for figure generation in a research publication remains somewhat novel and may warrant more detailed description of the validation process.

The STROBE checklist inclusion as supplemental material is good practice for cross-sectional studies.
